CHADIS is a web-based developmental screening, diagnostic and management system that administers and analyzes pre-visit, online questionnaires completed by parents, teens or teachers.

If you are interested in taking advantage of this service, before your child’s well visit, please log on to the CHADIS website and fill out the age appropriate screenings. We offer screenings for all ages. At the time of your child’s appointment, the practitioner will review and discuss your child’s screenings.

Please note: Chadis is optional and billable to your insurance. You may be liable for a deductible and/or coinsurance.

1. Go to the CHADIS.com website
2. New Users: click on “New Users Register Here” and Enter our Invitation Code: 3019073960
   
3. Already Registered: Enter your old user name and password
